RowdMap Expands Collaboration with CareMore Health to Improve Care Experience for Patients in Connecticut


LOUISVILLE, Ky., Nov. 29, 2017 — RowdMap, Inc. announced today an expanded collaboration with CareMore Health to develop high-value referral partnerships in support of CareMore Health's debut serving patients who are dual-eligible for Medicare and Medicaid in New Haven and Hartford. Through its CareMore @ Home model, CareMore Health is bringing coordinated care directly into the homes of high-risk patients in Connecticut.
