SEATTLE & LOUISVILLE, Ky.–Humana Inc. (NYSE: HUM), a leading health and well-being company, today announced it has collaborated with CHI Franciscan Health on an individual-focused model of health care for Humana Medicare Advantage members in Washington.
Known as value-based care, the model is designed to help people achieve their best health by emphasizing:
- More face time with physicians and personalized care that is tailored to each person’s unique health situation;
- Access to proactive health screenings and programs that are focused on preventing illness;
- Improved care for people living with chronic conditions with a focus on avoiding health complications (watch Humana’s “Better Chronic Management Through Value-Based Care” video);
- Leveraging technologies, such as data analytics, that connect physicians and help them work as a team to coordinate care around the patient; and
- Reimbursement to physicians based primarily on the health outcomes of their patients rather than solely on the number of services they provide (fee-for-service).
Humana’s Medicare Advantage plan members will now have in-network access to CHI Franciscan Health. In addition to offering patient services, the collaboration will focus on improving specific health metrics in Washington that are associated with high-quality care.
CHI Franciscan Health is a leader in quality care in the Puget Sound region. The system was recently awarded the only two “A grades” from Leapfrog in the South Puget Sound. With eight hospitals, nearly 300 clinics, and more than 12,000 employees, CHI Franciscan brings an innovative approach to patient-centric care that leverages partnership, technology, and science to keep patients and communities healthy.
“We are pleased to reestablish a relationship with Humana which will provide its members with access to CHI Franciscan Health care and furthers our commitment to manage the health and well-being of our patients,” said Dhyan Lal, CHI Franciscan Health Vice President Payer Strategy and Accountable Care.
Under this new agreement, CHI Franciscan Health’s physicians will also benefit by having new tools, access to real-time data and access to care support to maximize time spent with their patients covered by Humana Medicare Advantage.
“We are looking forward to expanding value-based care for our Medicare Advantage members in Washington by teaming up with CHI Franciscan Health,” said Catherine Field, Intermountain Region President for Humana’s Senior Products. “By integrating CHI Franciscan Health’s care delivery with Humana’s reporting tools and analytics, our goal is to provide our members with high-quality care that will help them get and stay on the path to good health.”
